If you have a permanent disability and require support to access the community, a Companion Card can help you with the costs of getting out and about with the support of a companion.
Receive a second ticket free of charge on public transport in South East Queensland, and at a range of participating venues and activities for the person who provides you with support. Your companion can be a carer, support worker, family member, volunteer or friend who supports you with mobility, communication, self-care or learning where you are unable to complete these tasks independently.
Companion cards are valid for five years and are issued to the person with a disability, not the companion or organisation who provides attendant care support. There is no age restriction for a Companion Card.
The NDIS is a separate scheme to the Companion Card program, therefore separate processes apply. To receive a companion card, you will need to complete an application in order to have your eligibility assessed.

To apply for a companion card, complete the application form here and attach two identical passport photos. Before you submit your application, you will need to have your details verified by a service provider or healthcare professional.
